A Note Regarding Medical Forms
All campers MUST
submit a Camp K'helah medical form, signed by a licensed physician,
prior to admission to camp. NO ONE WILL BE ALLOWED TO ATTEND CAMP
WITHOUT COMPLETEING THIS REQUIREMENT. This form must be returned
to the JCC office at least three working days prior to the camper's
start date. A separate form must be filled out for each child. Feel
free to photocopy forms if needed.
